Loyalty Card Terms & Conditions

 

  • This loyalty card is valid only for individual clients of Back to Fitness Physio and cannot be transferred or used by anyone else
  • The offer is only valid for full fee-paying patients and cannot be used in conjunction with any other offer or discount. 
  • Each visit entitles the cardholder to one stamp
  • The loyalty card does not extend to sports massage services
  • Once the card is fully stamped with the required number of visits (as specified on the card), the cardholder is entitled to the stated reward or discount, as per the current promotion
  • The completed card must be presented at the time of redemption. We will not accept any expired, damaged or altered cards.
  • Back to Fitness Physio reserves the right to modify or terminate this loyalty program at any time without prior notice.
  • The loyalty card has no cash value and cannot be exchanged for cash.
  • Lost or stolen loyalty cards will not be replaced
  • Loyalty card stamps are not retroactive and can only be earned from the date of issue
  • The FREE appointment must be redeemed within 5 months of date of issue
  • Applicable to self-funding patients only
  • Back to Fitness Physio will handle personal information collected for this loyalty program in accordance with our privacy policy.
By participating in our loyalty program and accepting this card, you agree to abide by these terms and conditions.
